I don't have a pacemaker but i have had heart surgery and was wired back together. The doctor gave me a card explaining it to present at Security. The one time i have flown since then i never had to show card went right thru. To be safe if i was you i would get something from your Doctor.

Tell TSA that you have an implant and they will send you through the scanner instead of walking through the metal detedtor. No big deal at all.
